Telehealth Informed Consent

Your care is delivered remotely and may be asynchronous ("store-and-forward"): a licensed clinician reviews the structured intake, health history and messages you submit, and may follow up by secure message, phone or video before making any clinical decision. You are not guaranteed a live video visit, though one may be offered or required where clinically or legally necessary.
Access to metabolic care without travel or waiting rooms; faster clinical review; care that continues between visits through messaging; and coordination among your clinician, pharmacy and care team in one place.
Information transmitted electronically may be insufficient for fully informed decisions (for example, the absence of a physical examination); technology can fail or delay care; security safeguards, while robust, can never make electronic transmission risk-free; and incomplete or inaccurate information you provide can result in adverse drug interactions or other harm. Telehealth may not be appropriate for every condition, and your clinician may direct you to in-person care.
Where clinically appropriate, your clinician may prescribe compounded medication prepared by a licensed pharmacy. Compounded drugs are made in regulated facilities but are not FDA-approved and have not been evaluated by the FDA for safety, effectiveness or quality; they may differ in appearance from product images. Your clinician will weigh risks, benefits and alternatives — including FDA-approved options — with you, and you may ask questions at any time.
You confirm that: the information you provide is accurate and complete; privacy laws that protect in-person care equally protect telehealth; alternatives to telehealth exist, including in-person treatment; results are not guaranteed; you will tell your other treating physicians about medications prescribed here; and you may access or receive copies of your records as described in the Notice of Privacy Practices.
You may withdraw this consent at any time without affecting your right to future care, by cancelling in your patient portal or emailing support. Withdrawal ends telehealth services going forward and takes effect alongside the Cancellation & Refund Policy; it does not undo care already provided.
Charges, renewals and refunds are governed by the Terms of Service and the Cancellation & Refund Policy shown at checkout — no separate financial obligations are created by this consent. If a clinician determines treatment is not appropriate, your payment is refunded in full automatically.
Prescriptions are dispensed by a licensed pharmacy and shipped to the address you provide. Verify your address carefully; report damaged or incorrect deliveries immediately so the pharmacy can replace them per the Refund Policy. Medication in transit must not be intercepted or redirected by anyone other than you.
